⚠️ Important: If your insurance plan requires a Primary Care Physician (PCP) assignment, please ensure Dr. Frederick Njuki is listed as your assigned PCP before your visit. If he is not currently assigned, contact your insurance provider to make the change prior to your appointment. This applies to HMO and PCP-required plans including Ambetter, Cigna/HealthSpring, and BCBSTX HMO.

New Patients
If you are scheduling as a new patient, please have the following ready when booking your appointment:
- Valid Photo ID
- Insurance Card
- Current Address
- Preferred Pharmacy (for prescription routing)
Please do not skip any requested information — incomplete submissions may delay your appointment confirmation.
Before Your Visit:
Once your appointment is confirmed, you can use the Healow app to submit your insurance information and upload a photo of your insurance card prior to your visit. Please plan to arrive 15 minutes early to complete any remaining intake forms at the office.
